Instructions
- Incomplete applications will not be accepted.
- Make sure to use your legal name as it appears in your social security card.
- Download the application from the link below, fill it out and print it. Bring the completed application to our office.
- Along with the completed application, also bring with you:
- Birth certificates of all household members
- Proof of income for all household members
- Court document for any child under the applicant's custody
- Marriage Certificate or Decree of Divorcement
- Social Security cards of all household members
- Florida Driver's License or Florida ID cards for all adults (18 years and older)
- A background report will be performed on each household member (18 years old and older)
